Reporting Requirements
Safety is a shared responsibility — Report it. Stop it. Fix it.
All near misses must be reported before shift end
No exceptions. An unreported near miss today is a real incident tomorrow.
Minor medical incidents require immediate supervisor notification
Any injury — however minor it appears — must be immediately reported to your supervisor and formally documented.
Never self-investigate — escalate to HSE
Report it, then step back. The HSE team conducts formal review. Self-investigation compromises evidence.
